Your Date Of Birth:		We ask this only to prove that you are eighteen years of age or older.
Your Company Name:		Leave this blank if you are a sole proprietorship.
Your Production Label:		The production name under which all of your films will be listed. We do not accept Production Labels including .com or .net or numbers.
Make Checks Payable To:		Enter the person's name or company name to whom we should send the revenue earned from sales of your films.
Social Security Number:		If you chose to have payments sent to a person then please enter that person's social security number here.
Federal Tax ID Number:		If you chose to have payments sent to a company then please enter that company's FEIN number here.
Payment Mailing Address:		Please enter the complete mailing address where we should send the revenue you earn through sales of your film. Be sure to include the zip code or postal code and the country.
Custodian of Records Name and Address:		Please provide the person's name who holds the model records for your products and the street address where those records can be found. We cannot accept company names or post office boxes or private mail boxes in this section. It must be a person's name and the physical street address where the documents are stored, even if it is your own private residence.
Your Phone Number:		Enter the phone number where we can call you between 8am and 4pm Pacific Time.
Your Fax Number:		Enter the fax number where we can fax you documents if needed.
Your Web Site URL:		Enter the URL for your web site.
Your Web Site Login:		If your web site is for members only then please provide us with a login and password so that we can review your site.
Recommended By:		How did you find out about AMVC? If another AMVC producer recommended you then tell us who it was so they can earn a bonus for bringing you to AMVC.
